Food or Environment Allergy-Intolerance Testing Form: Steps to hair testing diagnosis

• Please complete the allergy and intolerance testing form below and press submit. The form will come directly to our inbox.
  Note: If you have a problem with this then just print the form (using print form button at the bottom), fill in and send back with your hair sample and payment.
• Then cut a lock of hair (from the roots) of approx. 1 inch in length and .5  inch in width and place the hair sample in a small clean plastic bag and  write your name, address and phone number on the bag.
• Then all you need to do is post your hair sample, and a cheque for $120, made payable to Sheena Hendon (if you have not paid on line by Paypal) back to: Sheena Hendon Health, 773 Riddell Road, Glendowie, Auckland 1071, New Zealand We will then either email you the results or contact you in person. Results may take up to four weeks (but we aim for 2 weeks!).
• Depending on the results you may opt to book in for a follow-up consultation by phone or at the Sheena Hendon Health clinic to have you looking, feeling and being better than you ever  thought possible. If you have any questions please contact us
